NEW PATIENT INFORMATION

That Witch is dedicated to helping you reduce pain, improve performance, and get back to living your best, pain-free life! Our team is here to assist you every step of the way along your fitness and recovery journey. That includes the very first step—getting started as a new patient!

We understand that there may be some anxieties associated with receiving treatment and training, especially if you have never worked with That Witch before! That is why we have outlined everything new patients need to know and do to best prepare for their first visit.

WHAT TO EXPECT

Your first visit is all about getting to know you and your goals in order for us to create a treatment plan that best fits your condition and what you’re looking to get out of treatment. We’ll start the appointment with an in-depth discussion to learn about you and your condition. We will thoroughly review your relevant medical history and ask specific questions about your condition/issue.

Next, we will conduct the physical assessment in which we will guide you through a series of movements to assess joint mobility and muscle strength. Appropriate treatment intervention and overall plan of care will be based on the information obtained from the discussion and assessment in combination with your condition, specific needs, and goals. Once we establish your plan, we will work with you to create a flexible schedule that optimizes your treatments.

WHAT TO BRING

Please remember to bring the following to your first appointment with That Witch:

  • A valid, government-issued photo ID

  • Key past medical history, previous treatments, and current medications (in writing)

  • Any relevant paperwork from your doctor or other healthcare providers

  • Any assistive devices (brace, splint, cane, walker, or crutches)

We highly recommend bringing drinking water to stay hydrated during your session.

WHAT TO WEAR

What you wear to your first visit can impact our staff’s ability to conduct a thorough evaluation. For shoes, we request that patients wear or bring supportive, closed-toed athletic shoes (preferably sneakers). Regarding clothing, tight-fitting or restrictive outfits are not ideal; instead, we ask that you wear (or bring to change into) loose-fitting, comfortable clothing that permits you to move freely.

We request that patients wear/bring clothing that allows for easy access to the body part or affected area requiring treatment. If possible, patients being treated for their knee or ankle should wear shorts, and patients being treated for their shoulder should try to wear a sleeveless shirt.
